ANNEX 1 - MANDATORY CONDITIONS
1. No supply of alcohol may be made under the premises licence:
() at a time when here is no designated premises supervisor in respect of the premises licence, or
() at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
2. (a) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
(b) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
PREVENTION OF CRIME AND DISORDER
CCTV
The store shall operate a fully recordable CCTV system with the retention of images for a minimum period of 28 days for prosecution purposes.
THE PROTECTION OF CHILDREN FROM HARM
'Challenge 25'
Any person not looking the age of 25 must prove that they, as a minimum, are in fact over the age of 18 to purchase alcohol. Acceptable forms of identification concerning the issue of age are a Ten Year Passport, PASS logo proof of age card, HM Forces Warrant Card or photocard Driving Licence. All staff shall be trained to familiarise themselves with these requirements.
There shall be a due diligence policy incorporating the minimum Challenge 25 policy. All staff shall be trained on these policies including the operation of a refusal record and refresher training shall be provided on a regular basis".
There shall be prominent signage located throughout the store confirming the legal minimum age for the purchase of alcohol.
