ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
THE PREVENTION OF CRIME & DISORDER
Door Supervisors
Where any Door Supervisors are used at the premises they shall be registered with the Security Industry Authority.
Where any person is employed on or about the Premises as a Door Supervisor the Premises Licence holder shall maintain a daily signing log in a form approved by the Licensing Authority.
The daily signing in log for Door Supervisors may be in the form of a paper record containing details of the name, badge number, times on and off duty and signature of each individual or may be recorded electronically as part of the ID Scan (or similar electronic) system.
There shall also be maintained a nominal list of all door staff containing full name and address, date of birth, contact telephone details and where employed through an agency, the details of the agency. All records shall be maintained for a minimum of 2 years and made available for inspection by the police or an authorised officer.
The Premises Licence Holder shall keep a log of all incidents that occur on the Premises.
Door supervisors shall be required to wear a reflective jacket or waistcoat in yellow or orange and it shall be clearly labelled to identify the wearer as a door supervisor. The SIA badge shall be displayed either in a clear plastic holder on the chest or in a secure armlet.

Security Admission System
As soon as reasonably practicable and no later that 6 weeks from 17 March 2008 or such time as agreed with the police and the Licensing Authority, the Premises Licence holder shall install and maintain a membership, marketing and age verifier such as Club Scan or similar system together with a camera which will provide a full face photograph which will be stored and maintained by fully trained staff.
Dispersal Policy
The Premises Licence holder shall put in place a dispersal policy. The Premises Licence holder or Designated Premises Supervisor shall then ensure that this policy is complied with at all times when the premises are open for licensable activities.
CCTV
When CCTV is installed in the premises it shall comply with the Cheshire Constabulary Operational Guidelines for CCTV.
CCTV equipment shall be maintained in good working order in accordance with the manufacturer’s instructions and shall be operational at all times when a licensable activity takes place on the premises.
A written record shall be kept of images that are recorded by CCTV and shall include details of the recording medium used, the time and date recording commenced and finished. The record shall identify the person responsible for the recording and shall be signed by him/her. Where the recording is on a removable medium (i.e. videotape, compact disc, flash card etc), a secure storage system to store those recording mediums shall be provided.
The images recorded by the CCTV system shall be retained in unedited form for a period of not less than 31 days.
CCTV images shall be made available to the police or an authorised officer on request for crime prevention/detection purposes. The Premises Licence holder or Designated Premises Supervisor shall ensure that there are sufficient staff trained on the premises who can download CCTV images at the time of a request to maximise the effectiveness of any police enquiry to detect and prosecute offenders.
Pubwatch/Radio Scheme
The Premises Licence holder or their representative shall be an active member of the local pubwatch scheme and participate in the pubwatch radio scheme.
PUBLIC SAFETY
Occupancy
The total capacity for the premises shall be set at 150. A capacity for each of the 2 discreet rooms requires to be agreed and a mechanism put in place to manage the movement between them.
THE PROTECTION OF CHILDREN FROM HARM
Proof of Age Schemes - Challenge 21
There shall be in place for the premises a written policy that employees challenge all persons purchasing alcohol who appear to be under 21 years of age. That policy shall require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence, passport or accredited student card.
Persons who appear to be under the age of 21 years shall be required to produce proof of age by way of a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence, passport or accredited student card.
