ANNEX 1 - MANDATORY CONDITIONS
1) Supply of Alcohol
No supply of alcohol may be made under the premises licence;
() at a time when here is no designated premises supervisor in respect of the premises licence, or
() at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
CCTV
A close circuit TV system shall be installed and maintained. Images recorded and retained for at least 28 days and shall be handed to the Police/Local Authority on request.
Challenge 21
A proof of age scheme shall be in place.
'Think 21 Scheme' shall be in place and notices prominently displayed within the store.
Electronic till prompt shall be in place when alcohol is scanned through tills to remind staff to check the age of purchases.
A refusals book shall be kept to record sales which are challenged or refused.
Staff Training
all staff shall be trained in relation to underage sales of alcohol.
