ANNEX 1 - MANDATORY CONDITIONS
1) Supply of Alcohol
No supply of alcohol may be made under the premises licence;
() at a time when here is no designated premises supervisor in respect of the premises licence, or
() at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
2) Door Supervision
Only individuals licensed by the Security Industry Authority shall be used at the premises to guard against:
() unauthorised access or occupation (e.g. through door supervision), or
() outbreaks of disorder, or
(c) damage
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
The Prevention of Crime and Disorder
No stock can be removed from the site without correct documentation.
The Premise Licence Holder or his representative shall keep a record of all staff sales.
Access to the site is fully monitored by a security system.
The site shall be covered by CCTV cameras and shall be monitored by security staff.
The Protection of Children from Harm
No person under the age of 18 shall be admitted to the site.
